Aid Authorization
Federal regulations require that the University apply your Title IV Federal financial
aid funds (Direct Loans, Plus Loan, Pell, SEOG, etc.) first to "institutional charges",
including: tuition, mandatory fees, contracted University room and board fees, etc.
In order to apply any remaining Title IV funds to non-institutional (miscellaneous)
charges including, but not limited to, applicable book vouchers and health insurance
fee, the University is required to obtain the student's authorization. Furthermore,
authorization is required to apply funds, not to exceed $200, to a prior year charges.
If you have questions, please contact The Office of Student Financial Aid at (773)
995-2304. For additional information regarding the authorization regulation, please
visit the U.S. Department of Education's website at www.ed.gov.
Important
If authorization of Title IV funds is not authorized or provided, the student's account
will have an outstanding balance that must be paid immediately. If the balanced is
not paid, the student will not be eligible for a book voucher. Failure to pay in full
may result in a student's inability to register for future terms, receive a hold
release, etc. Also, service,. such as request for transcript and diploma, will be
restricted until the balance is paid in full. Note: all carryover outstanding accounts
are subject to a late fee.
Please be advised that failure to complete this authorization form will delay and
may impact the disbursement of your financial aid funds.
